Santé Care Center assists your Primary Care Doctor with completing an assessment of your health care needs once a year. This visit will be with a licensed nurse practitioner and they will: 

 

            Review and document all medical issues by completing a physical exam and asking about your medical history 

 

            Take a full record of all your medications 

 

            Complete a simple screening for peripheral artery disease (hardening of the arteries) 

 

            Complete an eye exam, if you are diabetic 

 

            Send your doctor a complete report  to assist with care planning
